The “Fit Person” Form

You are required to complete a “Fit Person” form at the time of submission of your application pack. This form consists of between 35-44 questions (depending on the particular version you are sent). LCS provides a training course which includes advice on completion of this form in our standard package.

Everyone must complete the section entitled CORE STANDARDS. Then you must complete the questions which are relevant to your particular service - these are called SERVICE SPECIFIC QUESTIONS. So, if you are operating only a laser clinic you do not have to complete the sections relevant to midwifery or termination of pregnancy - you can just ignore them!

If in doubt ask. The questions are all related to the management and administration of your business. None of them are in any way connected to how you do your treatments from a technical point of view.

The “Fit Person” Interview

Both the Registered Manager and the Responsible Individual are interviewed prior to granting registration. The interview is usually conducted by two Assessors (Inspectors) and can be carried out either at your clinic or more usually at the local CQC office.

You will be asked questions based on the answers you have provided on the form submitted with your application. You must be aware of the statutory notifications and the regulations applicable to your category of registration.

The interview can last anything from 20 minutes to (the record we believe) 4 hours!!

You will not be asked any questions relating to the actual treatments you provide but instead to the way in which yu will run and administer the service you are providing in terms of the paperwork, records, requirements and regulations - “paper not patients”!
